Check Incentives

Starting after the ASU transition, some internal Psychology Department information is also being maintained at a new Psychology Intranet site.

Checks and related documents that are misplaced, misdirected, stolen, or destroyed (intentionally or unintentionally) are very hard to replace. Please treat checks and related documents as though they are cash!

What follows are reminders of the procedure and policy for use of check incentives.

Requests

  • All requests require the Lab name (please use PI’s last name), Study Name, Funding Source (Index) and Activity Code of PI and need to be completed by Administrative Personnel
  • Address fields are required (on-line or on form) if the check disposition MAY include mailing. Requests with addresses that cannot be pre-verified against a national database will be returned to the requesting researcher for follow-up.
  • Checks are typically requested in one of the following two ways:
    • On-line (recommended when incentive amount is fixed and checks can be prepared in advance)
      • Incentive payments can be requested via the on-line form in Qualtrics (if you’ve forgotten the link, please request from incentives@uoregon.edu )
      • The On-line form allows submission of up to 5 participants at a time under the same funding source
    • On Research Incentive Check Request form (required when incentive amount is unknown or checks cannot be prepared in advance)
      • Check_Request(tall)
      • Corrections are NOT permitted in the amount field of the form, if a mistake is made in this field, void the entire form and start a new one
      • Fill out form legibly. Amount, address and name fields especially need to be clear and accurate. Requests with addresses that cannot be pre-verified against a national database will be returned to the requesting researcher for follow-up.
      • Check request forms must be turned in to PSY business staff (Jessica, Vonda, or George)
      • Checks are typically ready for pick up or mailing within two business days.
      • Checks mailed immediately will typically be received by participants within 5-10 business days.

Security

  • Distributed checks AND any associated documentation are the responsibility of the individual to whom they are distributed
  • Checks and documentation should be stored in a locked drawer or cabinet within a locked office or lab space
  • Checks and documentation should be transported in a container (i.e. an envelope or bag) within a container (i.e. a backpack or satchel)
  • Undistributed checks and documentation must be returned to the PSY business office

Restrictions

  • Held checks can only be picked up by the subject/guardian listed on the form and require the presentation of photo ID
  • Checks for payment of participation in studies for human subjects are made payable to adults only. If the human subject is a minor, checks are made payable to the subject’s guardian and this information must be provided.
